The case management electronic medical record (CM-EMR) provides case managers with disease-specific longitudinal data displays for individual patients and summary data displays across patient panels, data displays tailored for patients and secure messaging among patients and providers. The CM-EMR shares patient identification information with the main clinical information system (CIS). In addition, multiple data interfaces were required for transfer of data between the CM-EMR and the CIS. The CM-EMR allows patients view their own data uploaded from the HTU. Two different security solutions for the CM-EMR were developed: a PKI solution for patient access, and an LDAP-based solution for clinicians.
