Patient Access to the Web
Patients are able to access a web-based data display in order to review their own glucose, glycosylated hemoglobin, and blood pressure levels, treatment plan, and other aspects of their own clinical data (e.g. weight, diet, and lipid levels). Simplified graphical longitudinal displays are available for these clinical parameters. Patients receive individualized computer alerts based on parameters set by the nurse case managers.
